Last season's Premier League Player of the Season awardee Eden Hazard had recently expressed his desire to leave Stamford Bridge in the midst of rumors linking him to Real Madrid, citing his broken relationship with current Blues manager Jose Mourinho and the absence of opportunities with the club as his main reasons.

The Daily Express reported that Hazard, the reigning two-time Chelsea Player of the Year recipient, told French newspaper L'Équipe of his plan to exit the club next season because of a "fractious relationship with Mourinho and a lack of ambition" with the defending English champions.

It was also reported that the 24-year-old Belgian winger was recently reprimanded by Mourinho after "liking" a social media post that showed Los Blancos' interest in him. The post was a photograph of him with a caption that said "According to Spanish media Eden Hazard is on Real Madrid's radar for a 2016 transfer."

Earlier this week, rumors have swirled that the Portuguese coach benching him during the team's 2-0 victory over Aston Villa last Saturday was a direct result of Mourinho's disgust over the whole incident.

Hazard is currently struggling with his form at Chelsea, scoring only once in nine appearances in all competitions for the Blues so far this season, which is a far cry from his superb performance last term that also netted him the PFA Player of the Year distinction, as per Caught Offside.

If speculations of Cristiano Ronaldo leaving the Bernabéu this January or next season will come true, then expect Hazard to walk away from England and become the latest Galactico signing for Real team president Florentino Pérez soon.
