In Season 2 finale, The Nathan James reaches the ports along the Mississippi River where they attempt to distribute the cure to the civilians. However, some immune groups continue to prevent the distribution. The show ended with a massive cliffhanger wherein Dr. Rachel Scott was shot by an assassin and her fate still remains unknown.
"The Last Ship" Season 3 premiere picks up with episode 1 titled "The Scott Effect." The title suggests that it will be about Rachel Scott and fans will finally find out whether she is dead or alive.
According to Spoiler TV's description for episode 1, Tom Chandler, the caption of Nathan James, flies to Asia to learn more about a "possible mutation of the Red Flu" leading to another epidemic in the east. Whether the news is true or not remains to be seen. As for Mike Slattery and the remaining crew, they head to Southeast Asia to distribute the cure. However, things take an unexpected turn when a new threat surfaces.
