Defending Premier League champions Chelsea FC had been struggling in the ongoing 2015-2016 season as they begin September eight points behind current league leader Manchester City in the overall standings following their harrowing 1-2 defeat against Crystal Palace last Saturday.

With his team's current plight, José Mourinho is well aware that he needs to make improvements, or at least changes, in the lineup soonest. With that, here are some of the latest transfer rumors from Stamford Bridge.

John Stones Undergoes Stamford Bridge Medical: Recent reports went out that the 21-year-old English defender is scheduled for a medical at the Blues' facility in Fulham and that the announcement of the £40-million transfer is imminent.

Jones is just what Mourinho needs to further strengthen Chelsea's backfield that has given up nine goals in four games so far, which is just plain unacceptable.

However, latest reports emerged that Stones will stay put at Goodison Park as Mourinho settled for Senegal international Papy Djilobodji as the English transfer window closed. Stones continues to be a long-term target, though. Verdict: Fiction.

Victor Moses To Join West Ham: Blues midfielder Victor Moses is reportedly being moved to West Ham United this season on loan from Chelsea. The 24-year-old Nigeria international had been on loan to Liverpool and Stoke City for the last two years while still under contract with the defending champs.

It was reported that Moses will sign a new contract with Chelsea before he joins Slaven Bilic at the Boleyn Ground. Verdict: Fact.
